Disclosure Forms


The New York City Housing Development Corporation maintains a policy requiring a financial and personal review of all individuals and organizations with which it may conduct business.

Pursuant to that policy one of the following disclosure packages must be completed and returned to HDC (IMPORTANT – ONLY HDC FORMS CAN BE SUBMITTED. DISCLOSURE PACKAGES FROM OTHER AGENCIES ARE NOT ACCEPTABLE):  

  • Doing Business Data Form – THIS MUST BE COMPLETED BY ALL APPLICANTS.
     
  • Doing Business Data Form Q&A – FAQ's regarding the Doing Business Data Form.
     
  • Consent to Release Form – THIS IS TO BE COMPLETED BY ALL APPLICANTS. It allows the New York City Department of Housing Preservation and Development to release any pertinent disclosure material which they may have regarding the applicant.
     
  • Full Disclosure Package – this is to be completed if the applicant has never completed a full disclosure or has not completed one within the past twenty-four months;
     
  • Affidavit of No Change – this is to be completed if the applicant has submitted a full disclosure within the past twelve months and no significant changes have taken place;
     
  • Abbreviated Disclosure Package – this is to be completed if the applicant has submitted a full disclosure more than twelve months ago but less than twenty-four months and no significant changes have taken place;
     
  • Managing Agent Packages – this is to be completed by all applicants applying to be managing agents;
